Normally, when a man is sexually aroused, nitric oxide is released in the penis, which activates an enzyme called guanylate cyclase. This enzyme increases levels of cyclic guanosine monophosphate (cGMP), leading to relaxation of smooth muscles in the penile blood vessels, allowing more blood to flow into the penis. An erection is achieved when these blood vessels expand.
